Your Peak Advantage Member ID Card

Keep your member ID card handy when you get care. It’s your proof that you have a health plan. You’ll need to show it whenever and wherever you receive services, including at the pharmacy. If you have not yet received your Peak Advantage Member ID card, watch for it in your mail.

Create Your MyPeak Account

With MyPeak, 24-hour access to your account and personalized health information is easy and convenient
within our members-only web portal. Plus, there are easy-to-use online tools that allow you to:

  • View claim statements
  • Download plan-related documents

Partner With A Primary Care Doctor

Since your Peak Advantage plan is a PPO plan, you are not required to specify a primary doctor, but it’s recommended. Partnering with a trusted primary care physician or family doctor can be the best way to maintain and improve your health.

Make Your First Appointment

Schedule an annual wellness visit with your primary care doctor as soon as possible — especially if you’re
seeing a new doctor. Many preventive screenings and immunizations are covered at no cost to you, so it’s
good to get them on the calendar.

Bring your completed Health Risk Assessment with you to your appointment. It gives your doctor a snapshot
of your overall health and any health issues you may want to discuss.
