What’s a Medicare Advantage Plan EOB?
A Medicare Advantage Plan Explanation of Benefits (EOB) gives enrollees clear timely information about their medical claims and helps them make informed decisions about their health care options.
Peak Health, not individual providers, sends the EOB. The EOB is not a bill.
Your EOB will include, among other things, detailed payment information for all Peak Advantage Medicare Advantage plan claims processed during the reporting period. When you receive each EOB, you should:

Check for mistakes.
The easiest way to do this is to save your receipts and check them against the claims listed on the EOB. If you have questions, find mistakes, or want to file an appeal, you can contact Member Service.

Check your maximum out-of-pocket (MOOP) information.
Each EOB must show the dollar amount you have spent on cost-sharing in relation to the plan’s MOOP.
